Biological age test: what it costs and what the evidence says

A biological age test gives an "age" for your body that can be higher or lower than your real age. Many of these tests are epigenetic clocks. They measure chemical tags on your DNA, called methylation, in a sample of blood or saliva. A formula then turns the pattern of tags into an age. The gap between that age and your real age is the result (Genome Biology, 2019).
Different tests, different answers. There is no single agreed measure of biological age. There are many clocks, and each was built in its own way. The first clocks were built to estimate a person's age; later ones were built to predict health risks and death. So two tests can give the same person different results (The Scientist). Even one clock can vary on the same sample: a 2022 study found that testing the same sample twice could give results up to 9 years apart for six well-known clocks.

What the evidence says
Epigenetic clocks were developed as research tools. In large groups of people they are linked with health risks, and researchers hope they could one day show whether a treatment slows ageing. But a 2019 review in Genome Biology said it is not yet known how valid the different clocks are for one person. In 2024 a group of ageing researchers wrote in Nature Medicine that there is still no agreed way to check these tests before they are used in clinics.
This means it is not clear what a result should change. The tests have not been validated as a guide to treatment, and their use to judge whether a treatment works is still being studied. Ask the clinic what it would advise differently after the test, and talk to your own doctor before acting on a result.
